You must bring a valid government-issued ID and valid insurance card to utilize testing and counseling services.
How long is the in-person provider consultation?
The on-site provider will conduct a 5-10 minute consultation and intake to assess your health goals and provide health counseling based on your needs.
Is there any cost for the service?
